This keystone was returned by D. Ebel and A. White with the keystone missing in the jar and a broken micro fork. A thorough examination of the jar found the keyston flattened on the aluminum foil spacer that was used to stabilize the user mount in the jar. Info from D. Ebel and A. White: "#159 (C2117,1,159,0,0) was received before we went to APS in Feb 2011. We inspected it, and it was OK. When we arrived at APS, the keystone had fallen off the fork. The fork appears to have broken into pieces, and we can see the fork pieces. At no time did we remove the tape from the glass case. I cannot locate the keystone in the glass case. This case includes a foil shim that was used at JSC to compensate for imperfect machining of the holes through the glass into the aluminum block."
